Job Summary
The Clinical Manager is responsible for the day-to-day management of clinical operations within home care and hospice services. This role includes overseeing patient care, coordinating with clinical staff, and ensuring that programs meet organizational and patient care goals. You will work closely with the leadership team to foster an environment of high-quality service and continuous improvement.
Key Responsibilities
Team Leadership & Management
Conduct annual performance reviews and field supervision assessments for all clinical staff.
Ensure coverage and coordinate time-off for field staff as needed.
Oversee clinical orientation for new field staff in collaboration with the HR team.
Provide ongoing coaching and support for staff to ensure compliance with clinical standards and address any deficiencies promptly.
Clinical Operations
Collaborate with leadership to achieve financial goals for the home care and hospice programs.
Coordinate patient care services, including scheduling, care coordination, and assessments to ensure high-quality service delivery.
Assist in the development, implementation, and updates of individualized care plans for patients.
Ensure that patients' needs are continually assessed and addressed throughout their care journey.
Quality Assurance & Compliance
Partner with the Quality Director to monitor and improve the quality of care provided to patients.
Ensure adherence to agency policies, as well as State and Federal regulations, in all aspects of care delivery.
Oversee patient referrals and provide guidance on patient care needs as appropriate.
Help track and establish key quality indicators to meet or exceed national benchmarks.
Communication & Collaboration
Serve as a key point of contact for staff and patients, fostering clear communication between all parties.
Facilitate interdisciplinary team meetings and participate in case management discussions.
Respond to patient complaints by following established protocols, ensuring that all issues are addressed in a timely and professional manner.
Collaborate with other departments, such as HR and Finance, to ensure the smooth operation of the program.
Training & Development
Coordinate and provide training for new and current staff to ensure they meet the standards of practice.
Offer support and training on the use of Electronic Medical Records (EMR) systems and other tools required for patient care documentation.
Additional Responsibilities
Assist with triage calls and participate in the on-call nurse rotation as needed.
Monitor clinical schedules for consistency and ensure the efficient use of resources.
Oversee timecard management and the judicious use of overtime when necessary.
Support recruitment efforts by ensuring job postings are accurate and assisting with interviews.
Qualifications
Education & Experience
Associate's degree in Nursing required; Bachelor's degree preferred.
Previous management experience in a healthcare setting is preferred.
Active RN license in good standing in New Hampshire.
Must be fluent in English, both written and spoken.
Skills & Abilities
Strong leadership and communication skills.
Ability to manage difficult conversations with tact and professionalism.
Motivated to take on new challenges and provide excellent patient care.
Experience with Electronic Medical Records (EMR) systems and understanding of Medicare Conditions of Participation is a plus.
Other Requirements
Must be available to work weekends when needed.
Reliable transportation and valid driver's license required.
Must be able to meet physical demands, including occasional in-home visits and lifting of files.
Work Environment
This role operates in both an office setting and in the field for patient visits.
The position offers a mix of administrative tasks and direct patient interaction, with opportunities for remote work (with manager approval).
Physical Demands
While largely sedentary, some in-home visits are required.
Ability to lift files, open filing cabinets, and stand or bend when necessary.
